NAME CHANGES

Alabama Code §12-13-1

 

Adult Name Changes

 

* The Petitioner must be a legal resident of Coffee County and age 19 or older.

 

* The Petitioner must present picture identification, i.e., driver’s license.

 

* Signature of the Petitioner must be notorized.

 

* The filing fee is $21.00

 

Minor Name Changes

 

* Minor child must be legal resident of Coffee County.

 

* The petitioner must present a certified copy of the child’s birth certificate.

 

* The father listed on the birth certificate must petition jointly with the mother for a name change, or a sign a consent.

 

* The Petitioner(s) signature must be notorized.

 

* The minor child who is 14 or older must sign a consent to have a legal name change.

 

* The filing fee is $21.00
